At a law firm, pleadings for new clients are being put together from documents that should have been revised two years ago.

At an insurance company, a department is sending out letters with the old letterhead because no one has updated the template centrally – or at least no one noticed. And at a bank, the legal department is asking about the status of a regional clause. The answer requires three Outlook searches and a phone call to someone who is currently on vacation.

This isn’t bad luck. It’s a pattern.
